Hospice care combines the treatment of the body, the mind, and the soul to achieve a holistic model of care. Our hospice support is expanded to include loved ones and caregivers of the patient.

Palliative Medicine.

Palliative Medicine service promotes comfort and symptom management for patients that are still receiving curative care. Qualifying for this service does not require a terminal prognosis and is most beneficial when started at the diagnosis of serious illness.

A palliative care team is multidisciplinary. We work with patients, their medical providers, their family and caregivers to provide comprehensive support for people living with a serious illness such as heart failure, COPD, cancer, Parkinson’s Disease and many others.

The team of specialists including a physician, nurse practitioner, registered nurse, social worker, and nutritionist work together to help plan and provide emotional, social, and medical care. 

We are also able to help you navigate the medical system and to help you understand your choices in receiving medical treatment.
